Welcome to SP and I know you can make headway with your issues if you work hard to do what you can do...you and not someone else, so always keep your own unique situation in mind.
Now working at 10% at a time is the right way to go at it. Be as active as you can on your teams where I know you can find others who are also dealing with hypothyroid and sugar issues.
The exercise demos and videos here can be very helpful. You will find there are a number of exercise videos that do not put wt on the knees, even in the ones that do, you can always do some modifications. Now even without that and your total gym, you can always do some upper body exercises using cans from your cupboard as wts. Really doing what you can will help to boost your metabolism and as you have success you will find it easier too.
One final encouragement I would like to offer...those with lower income will not have to pay big penalties for not being able to buy insurance and I am hoping you will find that healthcare will become more accessable for you. Please don't allow yourself to get down about that until and unless you have to.
